The Insurance Mystery

Amjo can help with your insurance claims for UVA, UVB and UVB Narrow Band UVB EquipmentLet us help you file an insurance claim with your provider. There is no question, UV Phototherapy systems are expensive and many times your health insurance will cover all or part of the expense. We try to make this as painless as possible for our customers.

(A) We recommend that you take the first step and call your insurance company to determine if you have coverage for D.M.E. (Durable Medical Equipment). To speed up the call we recommend that you be prepared with the insurance codes for your disease and the code describing the equipment you are purchasing.

  • Diagnosis Code: (Click Here Now) This code tells the insurance support person what condition you are treating.

  • CPT/HCPCS Code: (Click Here Now) Sometimes called the "HicPic" (HCPC) code defines the equipment you are purchasing.

(B) You will need a prescription and it is strongly recommended that you also have a letter of medical necessity written by your doctor (Click Here For Sample Letters) which can help speed up the claim process. The prescription must be specific in that it must say "UVA", "UVB" or "UVB Narrow Band". UVB and Narrow Band UVB are different treatments and if the prescription only says UVB then we cannot provide Narrow Band UVB or vice versa.

(C) To help us file with your insurance company then we need some information from you before we begin. The information we need is as follows:

  • Details on your insurance company and personal details about the patient and of course the insured person. Click Here to provide this information.

  • A beneficiary assignment form which directs your insurance company to pay Amjo Corp. If you are pre-paying for the system in full then we do not need this form.

  • A prescription and/or a letter of medical necessity on your doctor's letterhead/stationery. This is an absolute requirement, we can do nothing until we receive your prescription or letter of medical necessity.
